> Regarding the commentary's phrase, "that is, the counteragent to the obstacle of diseases with poison-like characteristics due to the disharmony of the four great elements": this is from the text of the root treatise. From "expertly healing" onward, the commentary explains it using the four meanings from the Yogācāra\[-bhūmi\]: the first is the third, the upāya for eradication; the second is the fourth, non-recurrence once eradicated. Thus the Nirvāṇa \[Sūtra\] says: "What worldly physicians treat, though cured, recurs again; what the Tathāgata treats definitively never relapses." That healing and eradication is the third. The single word "expertly" corresponds to the fourth. From "madness to poisonous sorcery" onward, it distinguishes the first two meanings, first pointing to the sūtra generally, and later under "therein," separately assigning madness and the rest. From "internal four great elements" onward, all are the root treatise's explanation. Now, madness, mania, consumption, and emaciation directly explain the characteristics of disease, because the four great elements are disharmonized. Heaviness is the characteristic of earth disease; cold is the characteristic of water disease; yellow fever is the characteristic of fire disease; and wind characteristics are manifold. These two matters, madness and mania, are also causes of disease: because of madness and mania, one does not eat what one should eat, eats what one should not eat, does what one should not do, and so forth, thus giving rise to various diseases. Demonic possession and poisonous sorcery are directly causes of disease, because diseases arise dependent on demons and the like. They are also characteristics of disease, being diseases caused by ghosts and the like. From the phrase "the treatise sūtra speaks of incantations and medicines" onward, the commentary reconciles the treatise sūtra. The present sūtra lacks "incantations"; thus the treatise takes medicines and incantations as that which can cure, which is subsumed under the present sūtra's "expertly \[prepared\] remedies and medicines." The causes of disease and death are what is cured; through the power of incantations and medicines, one who ought to die does not die.
