It consists of 47 lines and is incised on both sides of a single plate. The first 7 verses and the seal are the same as in No. 3 above. It was issued from the \*Jaya-skandhāvāra\* at Vikramapura, and records grant of the village of Viḍḍāraśāsana in Beṭaddacaturaka in Paścimakhāṭikā of the Vardhamāna-\*Bhukti\* to Vyāsadeva Śarman by \*P.-Parama-nārasiṃha-Pb.-M.-Śrīmal-Lakṣmaṇasena\* in the 2nd year of his reign.
