大治五年庚戌十月二十六日乙未申時，藥師寺善明房得業奉讀受已了，覺印之。

On the twenty-sixth day, yiwei, in the hour of the monkey, in the tenth month of the fifth year, gengxu, of the Daiji era [1130 CE], the Tokugyō [monk] Zenmyōbō of Yakushi-ji respectfully completed the reading and reception, and Kakuin verified it.
