The Commentary says: This symbolizes the afflictions of the Peak of Existence. Non-Buddhists born there are called "hungry ghosts." Because it is the peak of the three realms of existence, it is called "upon their heads." Afflictions arising again is called "fires blazing." Lacking the food of orthodox teachings and the nourishment of Dharma water is called "hunger and thirst." Being oppressed by afflictions is like "heat." Constantly having no place of peace is called "zhouzhang." Zhouzhang has the meaning of circulating and wandering. Sinking back down into the five destinies is called "running about frantically." Some editions have the character zhang 障 [hindered], which has the meaning of terror and panic. In the metaphors explained above, the intent is to cause revulsion and dread through various changing names, which the wise should understand.
