纏繞，疑惑也。諸佛祕密靡不皆知，故云說藥。今聞諸法皆如來藏，名真藥現前。舉昔方便疑今真實，名不能分別。

'Entangle' means doubt and confusion. Not being ignorant of any secret of the Buddhas is why he is said to 'explain medicines.' Now hearing that all dharmas are the tathāgatagarbha is called 'the true medicine appears before you.' Citing past expedients to doubt present reality is called 'cannot distinguish it.'
