「大德一心念！我某甲，請大德為和尚，願大德為我作和尚，我依大德故，得受具足戒，慈愍故。」（三請已。）

“May the bhadantas single-mindedly pay heed! I, N.N., request the bhadanta to be my upādhyāya. May the bhadanta act as my upādhyāya; relying on the bhadanta, I shall be able to receive the upasampadā, out of compassion.” (Requested three times.)
