「大姊僧聽！此某甲，從和尚尼某甲，求受大戒。此某甲，今從僧乞受大戒，和尚尼某甲。某甲自說清淨，無諸難事，年歲已滿，衣鉢具足。若僧時到，僧忍聽：僧今與某甲授大戒，和尚尼某甲。白如是。」

"Elder sisters, let the saṃgha listen! This N.N. seeks to receive upasampadā from the upādhyāyikā N.N. This N.N. now begs to receive upasampadā from the saṃgha, with the upādhyāyikā N.N. N.N. herself declares that she is pure, without any obstructive circumstances, her years of age are complete, and her robes and bowl are complete. If the time of the saṃgha has arrived, may the saṃgha grant permission: the saṃgha will now confer upasampadā upon N.N., with the upādhyāyikā N.N. This is the jñapti."
