大德一心念！我某甲，今請大德為和上，願大德為我作和上。我依大德故，得受具足戒，慈愍故。（三說。答云：）可爾。（或云：）如是。（或云：）當教授汝。（或云：）清淨莫放逸。

"May the bhadanta focus his single-minded attention! I, named so-and-so, now request the bhadanta to be my upādhyāya; I pray that the bhadanta will act as my upādhyāya. By relying upon the bhadanta, I will obtain upasampadā, out of your compassion." (To be spoken three times. He replies:) "It is acceptable." (Or says:) "So it is." (Or says:) "I shall instruct you." (Or says:) "Be pure and not indolent."
