善男子聽！今是至誠時，實語時，今隨所問汝，汝當隨實答。汝不犯邊罪不？汝不犯比丘尼不？汝非賊心入道不？汝非壞二道不？汝非黃門不？汝非殺父不？汝非殺母不？汝非殺阿羅漢不？汝非破和合僧不？汝不惡心出佛身血不？汝非是非人不？汝非畜生不？汝非有二形不？汝字何等？和上字誰？汝年滿二十不？衣鉢具不？父母聽汝不？汝非負人債不？汝非奴不？汝非官人不？汝是丈夫不？丈夫有如是病：癩、癰疽、白癩、乾痟、癲狂病。汝今有此諸病不？（又須隨事，一一答「無」。）

"Good man, listen! Now is the time for utmost sincerity, the time for truthful speech. Now, whatever I ask you, you should answer truthfully according to what is asked. Have you not committed an antimavatthu? Have you not defiled a bhikṣuṇī? Did you not enter the path with a thieving intent? Have you not corrupted the two paths? Are you not a paṇḍaka? Have you not killed your father? Have you not killed your mother? Have you not killed an arhat? Have you not broken the harmonious saṃgha? Did you not, with evil intent, cause the Buddha's body to bleed? Are you not a non-human? Are you not an animal? Are you not dual-sexed? What is your name? What is your upādhyāya's name? Are you a full twenty years of age? Are your robes and bowl complete? Have your parents given their consent? Are you not in debt to anyone? Are you not a slave? Are you not a government official? Are you a man? Men have such diseases as: leprosy, carbuncles and ulcers, white leprosy, dry consumption, and madness. Do you now have these diseases?" (He must also answer "No" to each item one by one according to the matter.)
