我某甲，歸依佛，歸依法，歸依僧。隨如來出家，某甲為和上，如來、至真、等正覺，是我世尊。（三說。）我某甲，歸依佛，歸依法，歸依僧。隨如來出家竟，某甲為和上，如來、至真、等正覺，是我世尊。（三說。）授十戒相（語云：）

"I, named so-and-so, take refuge in the Buddha, take refuge in the Dharma, take refuge in the Saṃgha. Following the Tathāgata, I go forth, with named so-and-so as upādhyāya; the Tathāgata, the Arhat, the Samyaksambuddha is my World-Honoured One." (Spoken three times.) "I, named so-and-so, take refuge in the Buddha, take refuge in the Dharma, take refuge in the Saṃgha. Following the Tathāgata, I have gone forth, with named so-and-so as upādhyāya; the Tathāgata, the Arhat, the Samyaksambuddha is my World-Honoured One." (Spoken three times.) Conferring the marks of the ten precepts (Speak, saying:)
