2.58

大德僧聽！我某甲比丘，犯某僧殘罪，覆藏（若干）日。我某甲比丘，犯某僧殘罪，隨覆藏（若干）日，已從僧乞覆藏羯磨；僧已與我某甲比丘隨覆藏（若干）日羯磨。我某甲比丘行覆藏竟，已從僧乞六夜摩那埵；僧已與我某甲比丘六夜摩那埵。我今行摩那埵竟，願僧憶持。（三說。）

"May the venerable saṃgha listen! I, bhikṣu so-and-so, committed a certain saṃghāvaśeṣa offence and concealed it for [such-and-such a number of] days. Having committed a certain saṃghāvaśeṣa offence and concealed it for [such-and-such a number of] days, I, bhikṣu so-and-so, have already requested from the saṃgha the karman for concealment; the saṃgha has already granted to me, bhikṣu so-and-so, the karman for concealment corresponding to [such-and-such a number of] days. Having completed the practice of concealment, I, bhikṣu so-and-so, have already requested from the saṃgha the six nights' mānatva; the saṃgha has already granted to me, bhikṣu so-and-so, the six nights' mānatva. I have now completed the practice of mānatva. May the saṃgha keep this in mind." (Spoken three times.)
