大德僧聽！我某甲比丘，犯某僧殘罪，覆藏（若干）日。我某甲比丘，犯某僧殘罪，隨覆藏（若干）日，已從僧乞覆藏羯磨；僧已與我某甲比丘隨覆藏（若干）日羯磨。我某甲比丘行覆藏時，中間更重犯某僧殘罪覆藏（若干）日，亦從僧乞前犯中間重犯某僧殘罪覆藏（若干）日及覆藏本日治羯磨；僧亦與我某甲比丘前犯中間重犯覆藏（若干）日及覆藏本日治羯磨。我某甲比丘行前犯中間重犯覆藏（若干）日及覆藏本日治羯磨竟，已從僧乞六夜摩那埵；僧已與我某甲比丘六夜摩那埵。我某甲比丘行摩那埵時，中間更重犯某僧殘罪不覆藏，亦從僧乞前犯中間重犯某僧殘罪不覆藏六夜摩那埵及摩那埵本日治羯磨；僧亦與我某甲比丘前犯中間重犯某僧殘罪不覆藏六夜摩那埵及摩那埵本日治羯磨。我某甲比丘行前犯中間重犯某僧殘罪不覆藏六夜摩那埵及摩那埵本日治羯磨竟，今從僧乞出罪羯磨。願僧與我某甲比丘出罪羯磨，慈愍故。（三說。僧應與法。）

"May the venerable saṃgha listen! I, bhikṣu so-and-so, committed a certain saṃghāvaśeṣa offense and concealed it for [such-and-such a number of] days. Having concealed it for [such-and-such a number of] days, I, bhikṣu so-and-so, who committed a certain saṃghāvaśeṣa offense, have already requested from the saṃgha the karman for concealment; the saṃgha has already granted me, bhikṣu so-and-so, the karman for the [number of] days corresponding to the concealment. While I, bhikṣu so-and-so, was practicing concealment, I again committed in the interim a certain saṃghāvaśeṣa offense and concealed it for [such-and-such a number of] days; I also requested from the saṃgha the karman for the concealment for [such-and-such a number of] days of the previous offense and the interim repeated offense of a certain saṃghāvaśeṣa offense, as well as the karman of penance from the beginning of the concealment; the saṃgha also granted me, bhikṣu so-and-so, the karman for the concealment for [such-and-such a number of] days of the previous offense and the interim repeated offense, as well as the karman of penance from the beginning of the concealment. Having completed my practice of the karman for the concealment for [such-and-such a number of] days of the previous offense and the interim repeated offense, as well as the karman of penance from the beginning of the concealment, I, bhikṣu so-and-so, have already requested the six-night mānatva from the saṃgha; the saṃgha has already granted me, bhikṣu so-and-so, the six-night mānatva. While I, bhikṣu so-and-so, was practicing mānatva, I again committed in the interim a certain saṃghāvaśeṣa offense without concealing it; I also requested from the saṃgha the six-night mānatva for the previous offense and the interim repeated offense of a certain saṃghāvaśeṣa offense without concealment, and the karman of penance from the beginning of the mānatva; the saṃgha also granted me, bhikṣu so-and-so, the six-night mānatva for the previous offense and the interim repeated offense of a certain saṃghāvaśeṣa offense without concealment, and the karman of penance from the beginning of the mānatva. Having completed my practice of the six-night mānatva for the previous offense and the interim repeated offense of a certain saṃghāvaśeṣa offense without concealment, and the karman of penance from the beginning of the mānatva, I, bhikṣu so-and-so, now request the karman for rehabilitation from the saṃgha. May the saṃgha grant me, bhikṣu so-and-so, the karman for rehabilitation, out of compassion." (To be spoken three times. The saṃgha should grant the procedure.)
