大德僧聽！我某甲比丘，犯某僧殘罪，覆藏（若干）日。我某甲比丘，犯某僧殘罪，隨覆藏（若干）日，已從僧乞覆藏羯磨；僧已與我某甲比丘隨覆藏（若干）日羯磨。我某甲比丘行覆藏竟，已從僧乞六夜摩那埵；僧已與我某甲比丘六夜摩那埵。我某甲比丘行六夜摩那埵竟，今從僧乞出罪羯磨。願僧與我某甲比丘出罪羯磨，慈愍故。（三說。僧應與法。）

"May the venerable saṃgha listen! I, bhikṣu so-and-so, committed a certain saṃghāvaśeṣa offense and concealed it for [such-and-such a number of] days. Having concealed it for [such-and-such a number of] days, I, bhikṣu so-and-so, who committed a certain saṃghāvaśeṣa offense, have already requested from the saṃgha the karman for concealment; the saṃgha has already granted me, bhikṣu so-and-so, the karman for the [number of] days corresponding to the concealment. Having completed my practice of concealment, I, bhikṣu so-and-so, have already requested the six-night mānatva from the saṃgha; the saṃgha has already granted me, bhikṣu so-and-so, the six-night mānatva. Having completed my practice of the six-night mānatva, I, bhikṣu so-and-so, now request the karman for rehabilitation from the saṃgha. May the saṃgha grant me, bhikṣu so-and-so, the karman for rehabilitation, out of compassion." (To be spoken three times. The saṃgha should grant the procedure.)
