"Ayye, bear in mind with one intent! I, bhikṣuṇī [name], intentionally spoke falsely and committed [so many / numerous] prāyaścittika offences (the rest should be stated specifically according to the category and the matter). I now repent before the ayye, not daring to conceal them," and so forth, as above.
