切以今月嘉辰，菩薩應世奉為至尊皇后，報恩功德鎔寫靈像、度人出家，現在因緣福業無量。使五沙彌為弟子，輒當恭旨，作說戒師。年歲既小別須和尚，必選名德為作依止，並賜法衣，即於眾前歡喜頂戴。沙門某敬白。七月十九日。

I submit that on this auspicious day of the present month, the Bodhisattva has appeared in the world; on behalf of the Supreme Empress, for the merit of requiting kindness, wondrous images have been cast and people ordained into the sangha, bringing immeasurable roots of future merit and karmic fruits in the present. In making the five śrāmaṇeras become my disciples, I shall respectfully follow your decree and act as the precept-expounding master. Since their ages are young, an upādhyāya must be separately selected, and I will certainly choose an eminent and virtuous person to serve as their refuge, and bestow Dharma robes upon them, which they will joyfully receive atop their heads before the assembly. The monk so-and-so respectfully states. Nineteenth day of the seventh month.
