"I, the bhikṣu [name], with utmost sincerity make this vow: may my mind not be disordered at the end of life; may I directly go to be born in Sukhāvatī with correct mindfulness; may I behold Amitābha face to face and meet the assembly of sages, cultivating the ten stages and supreme constant bliss." (Having made the vow, bow to the Three Jewels. As for the method of making vows, the practitioner should, before Samantabhadra Bodhisattva and all the World-Honored Ones, mindfully contemplate and verbally express all vows held in the heart—namely: to practice the Way in this body without obstacle; that the four māras do not arise; to attain deep samādhi; to enter all dharma gates; to propagate the true Dharma and deliver sentient beings; to achieve correct mindfulness at the time of relinquishing life; and furthermore, in future lifetimes, always to meet the Three Jewels, be born in a family of right faith, leave home to cultivate the Way, make offerings to the Three Jewels, accept and uphold the Mahāyāna, have all the pāramitās appear before one, and together with all sentient beings achieve equal and correct enlightenment. In this way, in accordance with whatever various vows are held in the heart, all should be spoken from the heart as one wishes. Since this accords with the practitioner's personal disposition, it cannot be described in complete detail one by one.)
