With utmost heart I repent: I, monk [so-and-so], bow my head and take refuge in the Three Jewels of the ten directions and the three periods of time, the fundamental teacher Śākyamuni Buddha and others, the Buddhas, worthy ones, and sages invoked above, and Avalokiteśvara Bodhisattva of Great Compassion. With pity protect us, accept our invocation, manifest in the bodhimaṇḍa, and receive our repentance. I, monk [so-and-so], and others, on behalf of all sentient beings sinking in the painful destinies of the dharmadhātu, give rise to the bodhicitta and practice the bodhisattva path. Yet because the three karmas and six sense faculties have many encumbering sins, and the three obstacles are heavy, the bodhi practices and vows cannot be manifested, and we cannot benefit ourselves or benefit others, for which we deeply reprimand ourselves. Today we invoke the Buddhas, worthy ones, and sages, Avalokiteśvara Bodhisattva Mahāsattva of Great Compassion, and others, to repent of all evil karma of the past, present, and future lives, the three karmas, the six sense faculties, the three obstacles, and the three poisons. Ever since having a body, we have wandered in the six destinies, taking form everywhere, internally lacking the eye of wisdom, externally drawing close to evil people, opening the gates of laxity, committing the karma of birth and death; the branches and leaves pervade the three realms, and among the twenty-five states of existence there is none where we have not been born; turning in the cycle without end, continuous without cessation. By chance obtaining a human body, we have violated various grave precepts; of all violations, in both minor and grave categories and classes, there have been many infringements, contravening the pure and wondrous precepts of the Buddhas of the ten directions and three periods of time. If we do not repent, we shall fall into the Avīci Great Hell, the destinies of animals, hungry ghosts, and asuras, or among gods and humans, suffering immeasurable pain, transmigrating in the six destinies with no time of liberation. Today we adorn and purify the bodhimaṇḍa, sincerely repent, not daring to conceal anything. We pray only that the venerable Avalokiteśvara Bodhisattva Mahāsattva will manifest in the bodhimaṇḍa, emit supreme radiance, illumine and touch our bodies and minds, and make them pure, so that each and every precept root may be restored as before; that we may cause the Three Jewels to flourish, give rise to the mind protecting the Dharma, give rise to the mind protecting the precepts, give rise to the mind of the four means of embracing, and give rise to the mind of benevolence and patience; that our minds may be like vajra, and we vow never again to commit the sins already committed; that together with sentient beings of the dharmadhātu we repent; that to the exhaustion of future boundaries, we may constantly cultivate the bodhi practices and vows, in life after life constantly abide in pure buddha-lands, the three obstacles being eternally removed, the three evil destinies severed, forever departing from myriad sufferings, and achieving the unsurpassed path. Even if empty space has an end, my vows are inexhaustible; even if the nature of phenomena has a limit, the vow-mind is boundless, completely entering into the ocean of the Tathāgata's vow-nature. (Having repented and made vows, take refuge and pay homage to the Three Jewels.)
