如是我聞：一時佛在極樂剎大善法堂中坐。爾時無量壽如來告觀自在言：「一切眾生疾病短壽者，一切如來烏瑟膩沙最勝總持法門，若人受持讀誦，速得無病長壽安樂。」即時觀自在請佛宣說（呪廣不錄）。若人書寫此法，安置塔中、廣大供養，右繞千匝、恭敬禮拜，則獲智慧。若七日壽命延至七年，若七年壽命延至七十。獲得如是長壽安樂，無諸疾病，得宿命通。

Thus have I heard: At one time, the Buddha was seated in the Great Hall of the Good Dharma in the Land of Ultimate Bliss. At that time, Immeasurable Life Tathāgata addressed Avalokiteśvara, saying: 'For all sentient beings with illness and short lifespans, if someone upholds, reads, and recites the Dharma-gate of the supreme dhāraṇī of the uṣṇīṣa of all Tathāgatas, they will quickly attain freedom from disease, longevity, peace, and joy.' Immediately, Avalokiteśvara requested the Buddha to proclaim it (the mantra is extensive and not recorded here). If someone writes out this Dharma, places it in a stūpa, makes extensive offerings, circumambulates it clockwise a thousand times, and respectfully makes obeisance, they will obtain wisdom. A lifespan of seven days will be extended to seven years; a lifespan of seven years will be extended to seventy years. Having attained such longevity, peace, and joy, they will be free from all diseases and obtain the supernormal knowledge of past lives.
