4.16

「弟子某，至心敬禮日月光明世尊、火光如來、觀世音菩薩。乞念某每日持誦如來菩薩聖號各一百八遍，仰惟大慈大悲，各以願力令某早悟真性、增崇福力、無一切疾病災難等事。以上三者為救度眾生故。」禮拜。

'Disciple [so-and-so] sincerely pays homage to the World-Honoured One Sunlight and Moonlight Radiance, to Fire Radiance Tathāgata, and to Avalokiteśvara Bodhisattva. I beg that you be mindful of disciple [so-and-so] daily reciting the holy names of the Tathāgatas and Bodhisattva one hundred and eight times each. Looking up, I contemplate your great kindness and great compassion: may each, by vow power, cause me to awaken early to true nature, increase and exalt the power of blessings, and be free from all sickness, disaster, and difficulty. The above three are for the sake of saving sentient beings.' Make full prostrations.
