一各府州縣寺觀僧道並。從僧錄司道錄司取勘置文冊。須要開寫某僧某姓名年甲某布政司。某府某州某縣籍。某年於某寺觀出家受業某師。先為行童幾載。至某年某施主披剃簪戴。某年給度開報。

First: The monks and Daoists of monasteries and cloisters in all prefectures, departments, and districts shall have registers drawn up and examined by the Central Buddhist Registry and Central Daoist Registry. It must be clearly specified: which monk or which Daoist, full name, age and year-branch, which provincial administration commission, household registration of which prefecture, department, and district, in which year they left home to study under which master at which monastery or cloister, how many years they were first postulants, and in which year and under which donor they were tonsured and received robes/headwear, and in which year ordination was granted, all being reported.
