大德一心念。我弟子某甲。今請大德為懺悔阿闍梨。願大德為我作懺悔阿闍梨。我依大德故得懺悔。慈愍故（一遍亦得三遍彌善）。

"May the Venerable One be single-mindedly mindful. I, disciple [so-and-so], now invite the Venerable One to be my ācārya of repentance. May the Venerable One serve as my ācārya of repentance. Relying on the Venerable One, I am able to repent. Out of compassionate pity." (Once is acceptable; three times is even better.)
