91.12

某身所行惡。口所言惡。意所念惡。今已除棄。某若干日若干夜。受菩薩齋。自歸菩薩。

'The evil committed by my body, the evil spoken by my mouth, and the evil thought by my mind have now been cast aside. I, so-and-so, for such-and-such days and nights, undertake the bodhisattva fast and take refuge in the bodhisattvas.'
