tatra pradhāno vyādhiḥ, &vyādherguṇabhūta upadravaḥ, tasya prāyaḥ pradhānapraśame praśamo bhavati/

In that context, the primary disease is primary, and the complication is secondary to the disease; usually, upon the alleviation of the primary one, it too is alleviated.
