五者內外一雙。觀世音謂菩薩內德，潛眸密照即知病識藥。普門謂菩薩外德，種種方便充滿十方，救度眾生應病授藥也。

Fifth is the pair of internal and external. "Guanshiyin" refers to the bodhisattva's internal virtue: secretly observing with hidden gaze, he immediately knows the illness and recognizes the medicine. "Universal Gate" refers to the bodhisattva's external virtue: various skillful means fill the ten directions, saving and crossing over sentient beings, dispensing medicine in accordance with the illness.
