Second is according to the gate of transmigration: among what is to be remedied, this refers to delusion and craving initiating and moistening [rebirth]. From "severing all" on clarifies the antidote. Due to delusion and craving, there is transmigration in the various realms; from delusion there is craving, and because the bodhisattva's sickness arises, all sentient beings [exist in] the three realms of existence and the five destinies. The three fetters must certainly be completely severed: first, the various cravings of the desire realm are specifically named the fetter of desire; second, cravings of the upper two realms are collectively named the fetter of existence; third, the ignorance of the three realms is named the fetter of ignorance. The three fetters are also called the three outflows, which are the aforesaid delusion and craving. Once severed, the flow of saṃsāra is perpetually ended.
