問：通苦有四，別苦亦四。別苦苦於通苦，是則四種皆是苦苦，何故別說病為苦苦？答：生苦已過、死苦未至、老苦微薄，故約病苦說為苦苦。又解：淨名託疾，故但約病為苦苦也。

Question: General suffering has four types, and specific suffering also has four types. Specific suffering is more painful than general suffering, which means all four types are the suffering of suffering; why is disease specifically singled out as the suffering of suffering? Answer: The suffering of birth has already passed, the suffering of death has not yet arrived, and the suffering of old age is slight; therefore, the suffering of suffering is explained in terms of the suffering of disease. Another explanation: Vimalakīrti relied on illness [to teach], and therefore only disease is taken as the suffering of suffering.
