三中。明不善看病，因而致死；但無害意，故並結蘭。初與食破癰兩犯，並謂不合與而與。次不下，謂合與而不與。

In the third: it explains improperly nursing the sick, thereby causing death; however, because there is no harmful intent, they are all designated as sthūlātyaya. The two offenses of first giving food and lancing an ulcer both mean giving when it is inappropriate to give. Next, 'not...' means not giving when it is appropriate to give.
