Regarding multiple settlements: the two passages are self-explanatory. In the Mahāsāṃghika-vinaya, "the robe being separated from the head entails an offense" is because when standing up, the body is in a different settlement. (This differs from the reach of the cart and ladder in the Sarvāstivāda Vinaya Vibhāṣā.)
