Under The Intent of the Formulation, the first section: First, illness is described. Being "afflicted with many burdens" refers generally to all sufferings. "The four great [elements]," etc., specifically indicates the afflictions of disease: earth, water, fire, and wind pervade internal and external, so they are generally called "great"; the six viscera are the large intestine, small intestine, stomach, bladder, triple burner, and gall bladder. (From the crown to the heart is the upper burner, from the heart to the waist is the middle burner, and from the waist to the feet is the lower burner; these three together constitute one viscus.) "If" below next clarifies caring for them. "However" below third shows ordinary human sentiment. Nì means close. "Therefore" below reveals the sage's guidance. According to the Western Regions Record, there is a stupa to the northeast of Jetavana, which is where the Tathāgata washed a sick bhikṣu. When the Tathāgata was in the world, there was a sick bhikṣu dwelling alone in suffering. The Buddha asked, "What ails you? Why do you live alone?" He replied, "My nature is negligent and lazy; I could not bear to look after the sick. Therefore, now that I am afflicted with illness, no one cares for me." The Buddha pitied him and said, "Good man, I will now care for you."
