「大德僧聽！此某甲，從和尚某甲，求受具足戒。此某甲，今從眾僧乞受具足戒，某甲為和上。某甲自說清淨，無諸難事，年滿二十，三衣鉢具。若僧時到，僧忍聽：僧授某甲具足戒，某甲為和尚。白如是。」

"Bhadantas, may the saṃgha listen! This N.N. seeks to receive the upasampadā from the upādhyāya N.N. This N.N. now requests the upasampadā from the assembly of the saṃgha, with N.N. as upādhyāya. N.N. declares himself to be pure and free from all obstructive conditions, fully twenty years old, and equipped with the three robes and bowl. If the time of the saṃgha has come, may the saṃgha grant permission: the saṃgha confers the upasampadā upon N.N., with N.N. as upādhyāya. This is the jñapti."
