大德僧聽！我某甲比丘尼，犯某僧殘罪，已從僧乞半月摩那埵；僧已與我某甲比丘尼半月摩那埵。我某甲比丘尼已行摩那埵竟，今從僧乞出罪羯磨。願僧與我某甲比丘尼出罪羯磨，慈愍故。（三說。僧應與法。）

"Let the saṃgha, venerable sirs, listen! I, the bhikṣuṇī [so-and-so], committed [such-and-such] saṃghāvaśeṣa offense and already requested a half-month mānatva from the saṃgha; the saṃgha has already granted me, the bhikṣuṇī [so-and-so], the half-month mānatva. I, the bhikṣuṇī [so-and-so], have already finished undergoing the mānatva, and now request from the saṃgha the karman of rehabilitation. May the saṃgha grant me, the bhikṣuṇī [so-and-so], the karman of rehabilitation, out of compassion." (Three times. The saṃgha should grant the procedure.)
