大德僧聽！我某甲比丘，犯某僧殘罪，覆藏（若干）日。我某甲比丘，犯某僧殘罪，隨覆藏（若干）日，已從僧乞覆藏羯磨；僧已與我某甲比丘隨覆藏（若干）日羯磨。我某甲比丘行覆藏時，中間更重犯某僧殘罪覆藏（若干）日，亦從僧乞前犯中間重犯某僧殘罪覆藏（若干）日及覆藏本日治羯磨；僧亦與我某甲比丘前犯中間重犯覆藏（若干）日及覆藏本日治羯磨。我某甲比丘行前犯中間重犯覆藏（若干）日及覆藏本日治羯磨竟，已從僧乞六夜摩那埵；僧已與我某甲比丘六夜摩那埵。我某甲比丘行摩那埵時，中間更重犯某僧殘罪不覆藏，（此中且據重犯不覆藏者作法，下並同此。其重犯覆者合治，自入應治中辯。）今從僧乞前犯中間重犯某僧殘罪不覆藏六夜摩那埵及摩那埵本日治羯磨。願僧與我某甲比丘前犯中間重犯某僧殘罪不覆藏六夜摩那埵及摩那埵本日治羯磨，慈愍故。（三說。僧應與法。）

"May the venerable saṃgha listen! I, bhikṣu so-and-so, committed a certain saṃghāvaśeṣa offence and concealed it for [such-and-such a number of] days. Having committed a certain saṃghāvaśeṣa offence and concealed it for [such-and-such a number of] days, I, bhikṣu so-and-so, have already requested from the saṃgha the karman for concealment; the saṃgha has already granted to me, bhikṣu so-and-so, the karman for concealment corresponding to [such-and-such a number of] days. While I, bhikṣu so-and-so, was practicing concealment, in the interim I further repeatedly committed a certain saṃghāvaśeṣa offence and concealed it for [such-and-such a number of] days; I also requested from the saṃgha the karman of penalizing [sentencing back to] the original date for the concealment of [such-and-such a number of] days of both the earlier offence and the interim repeated offence; the saṃgha also granted to me, bhikṣu so-and-so, the karman of penalizing [sentencing back to] the original date for the concealment of [such-and-such a number of] days of both the earlier offence and the interim repeated offence. Having completed the karman of penalizing [sentencing back to] the original date for the concealment of [such-and-such a number of] days of both the earlier offence and the interim repeated offence, I, bhikṣu so-and-so, have already requested from the saṃgha the six nights' mānatva; the saṃgha has already granted to me, bhikṣu so-and-so, the six nights' mānatva. While I, bhikṣu so-and-so, was practicing mānatva, in the interim I further repeatedly committed a certain saṃghāvaśeṣa offence without concealing it (here the procedure is temporarily based on a repeated commission that was unconcealed; below is also the same as this. When repeated commissions with concealment are treated together, that is explained under the appropriate penalty), and I now request from the saṃgha the six nights' mānatva for both the earlier offence and the interim unconcealed repeated saṃghāvaśeṣa offence, and the karman of penalizing [sentencing back to] the original date for mānatva. May the saṃgha grant to me, bhikṣu so-and-so, the six nights' mānatva for both the earlier offence and the interim unconcealed repeated saṃghāvaśeṣa offence, and the karman of penalizing [sentencing back to] the original date for mānatva, out of compassion." (Spoken three times. The saṃgha should grant the procedure.)
