1.135

大姊僧聽！我某甲，從和尚尼某甲求受大戒。我某甲，今從僧乞受大戒，和尚尼某甲。願僧拔濟我，慈愍故。（三說。若受戒人不自稱名、不稱和尚尼名、教乞戒不乞戒、著俗服等、若眠、醉、狂、裸形、瞋恚、無心、身相不具、借他衣鉢、若無和尚尼、若多和尚尼、若眾僧不滿，皆不名受戒。）

"Listen, ayye saṃgha! I, named so-and-so, seek to receive upasampadā from the upādhyāyinī named so-and-so. I, named so-and-so, now ask the saṃgha for upasampadā, with the upādhyāyinī named so-and-so. May the saṃgha deliver me, out of compassion." (Spoken three times. If the ordinand does not state her own name, does not state the upādhyāyinī's name, is taught to request the precepts but does not request them, wears lay clothing, or is asleep, intoxicated, insane, naked, angry, devoid of awareness, physically deformed, uses borrowed robes and bowl, or has no upādhyāyinī, has multiple upādhyāyinīs, or the saṃgha is incomplete, in none of these cases is it considered receiving the precepts.)
