"Regarding 'The intrinsic nature of the associated cause... what distinction in meaning is there?':" this inquires about the difference between the two causes. The associated cause is narrower, being only mind and mental factors. The co-existent cause (sahabhū-hetu) is broader, extending to all conditioned phenomena. Whatever is an associated cause is definitively a co-existent cause; there are co-existent causes that are not associated causes, excluding mind and mental factors, namely the remaining conditioned phenomena. Here, the narrower is used to question the broader: If the intrinsic nature of the associated cause is precisely the co-existent cause, what distinction in meaning is there between these two causes?
