「弟子某甲等，仰啟盡虛空遍法界一切諸佛，今於此索訶世界，一四天下，南贍部洲，人主地某聚落某伽藍此道場中，有眾多菩薩，來於我所，三說求受菩薩戒。我已為作證，唯願慈悲亦為作證（禮拜三請）。」

"I, disciple so-and-so, reverently address all the Buddhas filling empty space and pervading the entire realm of phenomena: Now in this Sahā world, among the four continents, in Jambudvīpa, in this land of the sovereign, in such-and-such village, in such-and-such saṅghārāma, within this bodhimaṇḍa, there are many bodhisattvas who have come to my place, asking three times to receive the bodhisattva precepts. I have already borne witness for them; we only pray that you will also compassionately bear witness." (Prostrating and requesting three times).
