Furthermore, one contemplates that this illness is entirely due to cherishing body, life, and wealth, thereby suffering many afflictions; it is also that observing precepts was not complete, causing much illness and a short lifespan; it is also that the mind's resolve was weak, unable to bear patiently, so that the body's spirits did not offer protection; it is also that the power of vigor was slight, with no good deeds to repair or ward off misfortune; it is also that there was no power of dhyāna-samādhi, so that one was shaken by illness; it is also that the mind had little prajñā, failing to penetrate impermanence, suffering, emptiness, and non-self, thereby incurring this disease. Now, using one's own disease to sympathize with the diseases of others, one immediately gives rise to loving-kindness and compassion, bringing forth vows and practices. One gives without any remaining stinginess, patiently endures in accordance with principle, diligently applies correct mindfulness, and awakens to impermanence. This is giving rise to the Six Pāramitā Bodhisattva realm due to illness. Furthermore, contemplating this illness, one knows it arises from false imagination, inverted views, and the various afflictions of past lives. Such false imagination has no reality; both the self and nirvāṇa are empty. This is called giving rise to the Shared Teaching Bodhisattva realm due to illness. Furthermore, contemplating this illness, although it is ultimately empty, and emptiness has nothing to receive yet experiences various feelings; not yet having completed the Buddha-dharma, one should not extinguish feeling to realize cessation. This is giving rise to the Distinctive Teaching Bodhisattva realm due to illness. Dharmas such as these arise in succession due to disease and affliction; these are called the "conceivable realm", which is not what is to be contemplated now.
