弟子（某甲）一心奉請釋迦如來為得戒和尚。我依和尚故，得受菩薩戒。慈慜故。
"I, disciple [so-and-so], single-mindedly and respectfully invite Śākyamuni Tathāgata to be my preceptor from whom I receive the precepts. Relying upon the preceptor, I am able to receive the bodhisattva precepts, out of his compassionate mercy."
