龍朔元年。西華觀道士朝散大夫郭行真所造。真夙知希向。早預法流。形雖黃老心染緇釋。經像福本。每事經營。用資景業。通被存沒。必願罪終福始。惑盡智明。逮及黎元咸資敬仰。

In the first year of the Longshuo era [661 CE], made by Guo Xingzhen, Grand Master for Court Discussion and Daoist priest of Xihua Abbey. From early on, Zhen knew of aspiration and early entered the stream of the Dharma. Though his bodily form was of Huang-Lao, his mind was dyed by the black robes of Śākya. Scriptures and images are the root of merit; in every matter he applied his efforts. Using this to aid his glorious karma and comprehensively cover both the living and the deceased, he earnestly vows that offenses will end and merit begin, delusion be exhausted and wisdom shine bright, extending to all common folk so that they may all be supported in reverent devotion.
