請依止文　大德一心念。我比丘某甲。今請大德為依止阿闍梨。願大德為我作依止阿闍梨。我依大德故得如法住。慈愍故（三說）彼應報言。可爾。或言與依止。或言莫放逸（此三法隨作一語。皆得不勞具說）。

Text for requesting dependence: "Bhadanta, preserve single-minded mindfulness. I, the bhikṣu named so-and-so, now request the Bhadanta to be my ācārya of dependence (niśrayācārya). May the Bhadanta act as my ācārya of dependence. Relying upon the Bhadanta, I will be able to dwell in accordance with the Dharma, out of compassion." (Spoken three times.) The other should respond, saying: "It is well," or say: "I give dependence," or say: "Do not be heedless." (Using any one of these three expressions is acceptable; there is no need to speak all of them.)
