In the metaphor, "skilled physician" illustrates the Nirvāṇa Sūtra; "skilled in understanding the eight arts" illustrates fully possessing curative methods. Below reveals curative efficacy: first treating afflictions, and from "a woman giving birth" downward treating evil karma. Within treating afflictions, the phrases are distinct in two ways: first, clarifying that the scripture can treat; second, from "impoverished and foolish" downward, clarifying that the scripture forcefully treats. In the former, "in order to treat all the suffering of illness" brings out what is treated. "Giving various..." and so forth reveals the curative ability: giving various formulas confers its teaching methods; "medicines for vomiting and purging" confers the methods of practice, for practice can dispel obstacles, hence called medicines for vomiting and purging. "Anointing the body" gives rise to morality; "fumigating with herbs" generates concentration; from "nasal drops" downward clarifies the arising of wisdom: receiving the Dharma is "nasal drops", specific contemplation is called "powders", and general contemplation is called "pills".
