「大德僧聽！我某甲，從和尚尼某甲，求受大戒。我某甲，今從僧乞受大戒，和尚尼某甲。願僧拔濟我，慈愍故。」（三說已。）
"Bhadantas, let the saṃgha listen! I, N.N., seek to receive upasampadā from the upādhyāyikā N.N. I, N.N., now beg to receive upasampadā from the saṃgha, with the upādhyāyikā N.N. May the saṃgha rescue and deliver me, out of compassion." (After speaking three times:)
