大德僧聽！我某甲，從和尚尼某甲求受大戒。我某甲，今從僧乞受大戒，和尚尼某甲。願僧拔濟我，慈愍故。（三說。）

"May the Venerable Saṃgha listen! I, so-and-so, seek to receive upasampadā from upādhyāyikā so-and-so. I, so-and-so, now beg the saṃgha to receive upasampadā, with upādhyāyikā so-and-so. May the saṃgha rescue and deliver me, out of compassion." (Three times.)
