大德僧聽！我某甲，從和上尼某甲求受大戒。我某甲，今從僧乞受大戒，和上尼某甲。願僧拔濟我，慈愍故。（三說。）

"May the Venerable Saṃgha listen! I, so-and-so, seek to receive full ordination through the upādhyāyikā, so-and-so. I, so-and-so, now request full ordination from the saṃgha, with the upādhyāyikā, so-and-so. May the saṃgha deliver and aid me, out of compassionate pity." (Spoken three times.)
