弟子（某甲）一心奉請十方諸佛為尊證師，我依尊證師故，得受菩薩戒。慈慜故。
"I, disciple [so-and-so], single-mindedly and respectfully invite all the buddhas of the ten directions to be my venerable witnessing masters. Relying upon the venerable witnessing masters, I am able to receive the bodhisattva precepts, out of their compassionate mercy."
