1.95

大德僧聽！我某甲外道，歸依佛，歸依法，歸依僧。我於世尊所求出家為道，世尊即是我如來、至真、等正覺。（三說。）我某甲外道，歸依佛，歸依法，歸依僧已。從如來出家學道，如來是我至真、等正覺。（三說。）（次與戒相，與法同上。）

"May the venerable saṃgha listen! I, the non-Buddhist named so-and-so, take refuge in the Buddha, take refuge in the Dharma, take refuge in the Saṃgha. I seek to go forth and practice the Path in the presence of the World-Honoured One; the World-Honoured One is indeed my Tathāgata, Arhat, Samyaksambuddha." (Spoken three times.) "I, the non-Buddhist named so-and-so, have taken refuge in the Buddha, taken refuge in the Dharma, taken refuge in the Saṃgha. I have gone forth to learn the Path from the Tathāgata; the Tathāgata is my Arhat, Samyaksambuddha." (Spoken three times.) (Next, conferring the precept marks and the ritual method are identical to the above.)
